We had a vision for remote patient monitoring but no technical team to build it. 1Raft took us from whiteboard to 25 clinics in under four months - and the platform was HIPAA-compliant from day one.

The Challenge

What we were up against

Older adults and post-surgery patients required frequent in-person visits for routine vital sign checks - costly for providers and burdensome for patients, especially those with mobility limitations or living far from their clinic.

Patient-provider communication was fragmented across phone calls, fax, and disconnected EHR portals. Providers had no real-time visibility into patient health between visits, limiting their ability to intervene early when conditions deteriorated.

Manual administrative tasks - scheduling, documentation, insurance compliance tracking - consumed clinical staff time that should have been spent on patient care, while increasing the risk of errors and missed follow-ups.

Continuous monitoring devices existed, but no unified platform aggregated readings from multiple device types into a single provider view with prioritized alerts and historical trend analysis.
